Coenzyme Q10
Coenzyme Q10 is generally well tolerated, and its best-supported use is as an add-on in chronic heart failure, where one trial of 420 people found fewer major cardiovascular events, but the wider evidence is rated weak and stronger trials are still wanted. It does not reliably relieve statin muscle pain (meta-analyses disagree), lowers systolic blood pressure by only about 3 mmHg, and is rated only 'possibly effective' for migraine prevention.
If you take warfarin, tell your clinician first.

Evidence is rated per use. The same ingredient can be strong for one thing and traditional for another.
| Use | Evidence | What the research shows |
|---|---|---|
| Chronic heart failure, add-on to standard careCompare remedies for heart health → | Moderate | Q-SYMBIO (420 people with moderate to severe heart failure, 2 years, 100 mg three times a day) found no change in the 16-week symptom endpoints, but major adverse cardiovascular events were 15% versus 26% on placebo (hazard ratio 0.50, 95% CI 0.32 to 0.80). A York systematic review of 26 trials (2,250 people) found a possible but not clear-cut fall in all-cause mortality (relative risk 0.68, 0.45 to 1.03) and more modest short-term results. It judged the trials often at high risk of bias and said stronger evidence is needed before NHS prescribing; NCCIH calls the heart-failure research inconclusive.3,4,9 |
| Male infertility (idiopathic), sperm qualityCompare remedies for men’s health → | Moderate | A 2025 meta-analysis of 9 randomised trials (781 men) found higher sperm concentration (10.2 million per mL), motility (about 5 percentage points) and clinical pregnancy odds (odds ratio 6.02, 95% CI 1.97 to 18.41, a very wide interval). The trials were small and the pregnancy result rests on few studies; NCCIH says the research is too limited for conclusions. Mostly studied in men, not for female fertility, where we found no source we could cite.7,9 |
| Migraine preventionCompare remedies for headaches → | Limited | The 2012 American Academy of Neurology and American Headache Society guideline rated CoQ10 'possibly effective' for migraine prevention (Level C), below magnesium, riboflavin and feverfew (Level B). The evidence is a few small trials; this is a modest, low-risk option, not a first-line one.5 |
| Statin muscle pain | Limited | Two meta-analyses of similar small trials disagree. One (7 trials, 321 people) found no benefit over placebo (mean pain difference -0.42, 95% CI -1.47 to 0.62) and no better staying on the statin; a 2025 analysis (7 trials, 389 people, 30 to 90 days) found a small pain reduction (-0.96, -1.88 to -0.03), with three of the seven trials showing no change. NCCIH says the overall evidence does not support it.1,2,9 |
| Lowering blood pressure | Limited | A 2025 meta-analysis of 45 randomised trials found systolic pressure fell by 3.4 mmHg (95% CI 1.6 to 5.1), with no significant change in diastolic pressure or heart rate; the effect was larger below 200 mg a day and beyond 8 weeks. That is small. NCCIH judges the effect probably not meaningful.6,9 |
| Wrinkles and skin ageing, topicalCompare remedies for skin → | Traditional | A 2024 narrative review reports that CoQ10 creams raise skin CoQ10 and reduce wrinkle depth, but it searched one database, pooled no data and the author discloses consulting work for many skincare companies. We found no controlled trial we could open and cite, so this stays at traditional-level evidence.8 |

How people take it
Safety
Who should avoid
- Warfarin — case reports of reduced effect; check with your clinician first
- Insulin — NCCIH says CoQ10 may interact; ask your clinician
- Cancer treatment — may not be compatible; ask your oncologist
- Blood-pressure medicines — may add to the effect, so pressure may drop further
- Pregnancy and breastfeeding — we found no safety data

Questions people ask
What are the benefits of coenzyme Q10?
The best-supported is as an add-on in chronic heart failure, where Q-SYMBIO found fewer major cardiovascular events over 2 years, though a systematic review calls the wider evidence weak. It may modestly improve sperm quality and lower systolic blood pressure by about 3 mmHg. It is rated only 'possibly effective' for migraine prevention.
Does coenzyme Q10 help statin muscle pain?
Unclear. One meta-analysis of 7 trials found no benefit over placebo, another of 7 trials found a small pain reduction, and NCCIH says the overall evidence does not support it. If you have muscle pain on a statin, talk to your prescriber rather than stopping it yourself.
What are the side effects of coenzyme Q10?
NCCIH reports no serious side effects, and mild ones such as insomnia or digestive upset. The main caution is interaction with warfarin and insulin, and a possible clash with some cancer treatments.
Is coenzyme Q10 good for skin?
Creams are sold to reduce wrinkles, and a review says they do, but it was written by an author with industry ties and is not controlled trial data. We could not find a trial we could cite, so treat the claim as unproven.
